The Notice of Privacy Practices describes how the practice may use and disclose your protected health information, and explains your rights — including your right to inspect and obtain a copy of your record, to request corrections, to request restrictions on certain uses, to receive an accounting of disclosures, and to file a complaint.

You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalised for filing a complaint.
