What this website collects, and what it does not.
Draft for review. This policy describes how the website is actually built. It has not yet been reviewed by the practice or its legal adviser, and it must be before launch. It is not a substitute for the practice’s HIPAA Notice of Privacy Practices, which covers patient health information and is a separate document.
What this website collects
This site is a set of static pages. It does not run a content management system, it does not have user accounts, and it does not ask you to log in. There is no database of visitors.
The site loads no third-party scripts. It does not use advertising trackers, social media pixels, session recording, or cross-site profiling, and its Content Security Policy prevents any such script from running even if one were added by mistake.
Cookies
This site sets no cookies of its own and uses no analytics cookies.
If you contact us through this site
Where an appointment request form is available, it collects only what is needed to call you back: your name, your phone number, optionally your email address, the reason for the visit, and when you are free. It does not ask for, and you should not send, medical details, diagnoses, test results, insurance identifiers, or your Social Security number.
An online form and an email are not secure channels for health information. Anything clinical should be discussed by phone or at your appointment.
Calling and texting
Calls and text messages to the practice are handled by the practice’s telephone service, not by this website. Standard message and data rates may apply to texts.
Links to other sites
Where this site links out — for example to a mapping service for directions — that site has its own privacy practices, which we do not control.
Children
This website is intended for adults and does not knowingly collect information from children.
Changes
If this policy changes, the revised version will be posted on this page.
