Completing paperwork before you arrive means more of the appointment is spent on your hearing.
New patient forms are not available for download yet.
The previous intake form was hosted by the practice’s former website vendor and is no longer available. New forms will be published here once the practice has approved them and confirmed where completed forms should be sent.
In the meantime, paperwork can be completed at the office. Arriving a few minutes early is enough.

- Photo identification
- Your insurance card, if you have coverage
- A list of your current medications
- Your current hearing aids, with charger or batteries
- Any previous hearing test results you have
- A family member or friend, if one can come with you
Please bring medical information with you rather than sending it by email or text. Neither is a secure channel for health information.
